School Nurse
  • Please note that if you indicated on the Health Assessment portion of Registration that your child will need a medication or inhaler at school, a “School Medication Prescriber/Parent Authorization” form must be completed by a physician. This form is available from your physician.

  • All medications must be delivered by the parent to the school nurse. DO NOT send medications to school with your child. Students are not allowed to carry medications “on person” unless checked off by the school nurse and the physician has checked “self carry” on the authorization form.

  • If your child needs a PRESCRIBED MEDICATION while at school, a parent must bring the medication and signed authorization form from the physician to the school nurse. The authorization form must by signed by the physician and parent. Prescription medications must be brought in a current pharmacy labeled container with the student’s name, physician name, name of medication, strength, dosage, frequency, and route. The label must match the authorization form.

  • If your child needs an OVER THE COUNTER MEDICATION while at school, a parent must bring the medication to school. A parent can sign an authorization form that will be good for 2 weeks. This form is available from the school nurse. All medications must be brought to the school nurse in the original, unopened container. Medications cannot be brought in plastic bags or envelopes.

  • If your child has any food allergies, please have a physician complete a “Diet Prescription” form and turn into the school nurse. The form must be signed by both the physician and parent.
